Acceptable Forms of Identification


One (1) Primary or Two (2) Secondary IDs are required:

Primary Photo ID is required to establish proof of the identity of the person requesting a protected vital record. Acceptable forms of Primary Identification are:

Must Provide Photocopy of One (front and back)

  1. Government Issued Driver's License
  2. Military ID
  3. State ID card
  4. Valid Passport
  5. Government Issued Visa
  6. Permanent resident card
  7. Alien registration receipt card
  8. Employment authorization card
  9. Re-entry permit
  10. Refugee Travel Document
  11. VA Card (with intact photo)
  12. Voter's registration card (Countries outside of the U.S.)
  13. Certificate of Naturalization (with intact photo)
  14. Resident Alien card
  15. Concealed Carry handgun license

If a Primary Identification is not available, two Secondary Identification items of different types may be used to establish proof of identity. Acceptable forms of Secondary Identification are:

Must Provide Two

  1. Temporary Driver's License
  2. Social Security card (must be signed by card holder)
  3. Bank Statement with Requestor's current address
  4. Car Registration or Title with Requestor's current address
  5. Utility Bill with current address of Requestor and company letterhead with company name and address; not handwritten
  6. Current Pay Stub (must include your name, social security number plus name and address of business; not handwritten)
  7. Valid insurance card or policy of Requestor
  8. Valid health insurance card or policy of Requestor
  9. Parole document (book sheet) of Requestor
  10. Bureau of Indian Affairs Tribal ID card of Requestor
  11. Inmate ID of Requestor(along with a memo completed and signed by a counselor or parole officer)
  12. Filed Income Tax of Requestor with current address
  13. Letter to Requestor from Social Service Agency/Health Department or other government agency with current address
  14. Hospital or Health agency bill (with current address) of Requestor
  15. Court Documents of Requestor
  16. W-2 from Employer (with Requestor's current address)
  17. Letter from employer (with Requestor's current address)
  18. U.S. Voters registration card of Requestor
