1. Print and fill out the necessary form(s) here
2. Mail the form(s) to the appropriate vital statistics unit, which will be listed on the application form, along with your payment (check or money order)

Certificates

• Birth: $10 (additional copies are $10 each)
• Death: not currently available

Our transaction fee starts at $14, but may vary depending on the delivery method.

Kentucky is an open record state; no additional identification is required.

There are no disqualifying relationships or age limits. Anyone that has the correct information (as it is listed on the record) can apply.

• Standard: most orders will be delivered within 5 - 15 Business Days of receipt.
