There are three ways to order a birth certificate:

1) Online

1. Fill out the online order form
2. If you are requested to provide any supporting documentation after checkout, please do so here

2) By mail (15-25 Business Day Processing)

1. Locate, print and complete the appropriate form(s) which can be found here
2. Mail the application to the appropriate Vital Statistics Unit, which is listed on the form you fill out, along with your identification and payment (check or money order)

2. Bring an acceptable form of identification
3. Payment (cash, check, money order, credit/ debit card)
