Acceptable Forms of Identification

Official documentation is required to establish proof of identity of the person requesting a vital record. If requested, please provide one of the following:

1. One (1) item from Group A

or

2. Two (2) items from Group B, as well as the signed Statement from Requestor Form D1123F

Group A: Primary Identification

Items in Group A must be photocopies of the individual’s valid government-issued photo ID verifying the name and current mailing address. Expired IDs cannot be accepted. If the address has been changed, include a copy of the update card.

Examples:

1. State issued Driver License
2. Federal or State Identification Card
3. Learner's Permit
4. Concealed Handgun License
5. Pilot's license
6. Federal, State or City Law Enforcement Employment Identification Card

Group B: Secondary Identification

Two (2) items of different types within Group B are required to establish proof of identity. You must also submit a signed copy of the Statement from Requestor Form D1123F when submitting items from Group B.

2. Car Registration
3. Pay Stub
4. Bank Statement
5. Copy of Income Tax Return/W-2 Form
6. Lease/Rental Agreement
