  • Oklahoma Self-Defense Act (SDA) License / OK Concealed Carry Permit

  • Temporary Resident Card (Form I-688)
  • You need an Oklahoma secondary school ID. This applies to public, private, or parochial schools. The student must be 19 or younger. The ID must include the student's name, school name, date, and school contact information.
  • Oklahoma institution of higher education ID (must include name, school, date, and school contact info)
  • Oklahoma technology center school ID

Secondary IDs (submit TWO, legible photocopies)

At least one must show your current mailing address.

  • Pay stub or W2
  • Bank statement (redact account number and balances) and/or utility bill with name and current address
  • Social Security card (must be signed) or SSA record earnings statement with current address
  • US Selective Service Card
  • Official police report or arrest record with name and current address
  • School transcript
  • Non-Oklahoma state-issued temporary ID
  • Oklahoma lifetime hunting or fishing license
  • Marriage certificate (certified English translation if applicable)
  • Car registration, title, or insurance/security verification form with current address
  • Government benefit statement (DHS, WIC, SSA) showing name and current address

Note: If the person in the record is in prison, apply through the DOC Case Manager. Include a letter on facility letterhead that confirms their residency.
