Group A — Primary ID

Must provide one (1) of the following:

• Non-expired State-issued Driver License
• Non-expired Identification Card

• Learner's/Instruction Driver's Permit (if over 18 years of age)
• Pilot's License
• Concealed Weapons License
• Nonimmigrant Visa
• Permanent Resident Card (Form I-551)
• Temporary Resident Card
• Department of Justice - Immigration and Naturalization, Form I-197
• Certificate of Naturalization
• Nonimmigrant Visa/Border Crossing Card
• Military Common Access Card

Group B — Secondary ID

If a primary ID cannot be provided, must provide a minimum of three (3) of the following:

• Car Insurance Card (cannot be used with any other car-related documentation)
• Car Registration (must be valid; cannot be used with any other car-related documentation)
• Car Title (cannot be used with any other car-related documentation)
• Consular Report of Birth Abroad of a Citizen of the USA, FS240
• DMV Receipt for Fines Paid (must be recent)
• Employment ID that provides a description rather than a photo, with notarized statement from employer confirming current employment
• Employment Photo ID with notarized statement from employer confirming current employment
• Health Insurance Card
• Life Insurance Policy
• Medical Marijuana Card
• Marriage License
• Medicaid or Other Social Services Form or Identification
• Property Tax Bill
• School Photo ID with notarized statement from school confirming current enrollment
• Social Security Card
• Temporary Driver's License (description only, no photo)
• Traffic Citation (must be current)
• US Department of Homeland Security Form I-94 (Arrival/Departure Record)
• US Military Discharge Papers, DD214
• US Selective Service Card
• Veterans' Administration (VA) Card
• Voter Registration Card
• W-2 Form (within past 3 years)
• Bill or piece of mail addressed to applicant at provided address (only one bill may be used)
• Foreign Driver's License (may be used as one of the three secondary documents)
• Consular ID Card, Matricula, or Similar Card
