Instructions for Submitting Additional Items


Application:

  2. See below for instructions on how to submit your completed document.

Identification:

  1. You may use your current and valid driver license, identification card, or passport to provide as proof of identity. However, if you are unable to provide one of these common items, please see this complete list of acceptable forms of identification.
  2. See below for instructions on how to submit your Identification to us.

Using a Scanner:

  1. Scan the items using your scanner. Please make sure that the entire page is legible and that the quality of the image is sufficient to print.
  2. From your computer: follow this link. Please enter your email address to upload your documents. You may also email them to [email protected].

Using a Smart Phone or Digital Camera:

  1. Take a picture of your items using your smart phone or digital camera. Please make sure that the entire page is legible and that you are using the highest resolution setting available. Please take the image in bright light without the flash creating a glare. The entire image of each item must be viewable.

  2. From your computer: follow this link. Please enter your email address to upload your documents. You may also email them to [email protected].
